IMPORTANT NOTICE REGARDING FORM 1095-C

In accordance with federal law under the Paperwork Burden Reduction Act, Greenfield Products, Inc. will not automatically mail Form 1095-C to full-time employees unless a copy is requested. Your Form 1095-C contains information about the health insurance coverage offered to you by Greenfield Products, Inc. and may be retained with your personal tax records.

Employees may request a copy of their Form 1095-C at any time by contacting:

Requests may also be submitted in writing or by email. Upon receipt of a request, a copy of your Form 1095-C will be furnished to you within thirty (30) days.
